Several conditions affecting the frontal lobes can show up first as new disinhibition, poor judgement or a change in personality, and some are treated as psychiatric problems for years before anyone thinks of the brain itself. If a person who was fine at forty is unrecognisable at fifty five, that is a referral. Far better to have looked and found nothing.
If you do not know, ask people who know the Scripture (Quran 21:7). Passing a question on to someone who knows more is the practice itself, not an admission of failure.

In a large multicentre trial, methylphenidate outperformed placebo in adults with ADHD, while structured group psychotherapy was not clearly better than individual clinical management. Treatment does real work when the diagnosis under it is right, which is exactly why a hasty diagnosis is worth avoiding. That trial tested treatments rather than diagnostic accuracy, so it makes the case for care without measuring the cost of getting it wrong.

Strength of evidence: one line of research. A single study, or a theoretical account.
Symptoms that are new in an adult deserve a physical explanation before a psychological one.
Assess, make a plan, do the work, and then keep measuring while you do it. Without that last part you have no way of knowing whether to push harder, change direction or stop.
A clinician trained thoroughly in autism and ADHD may know almost nothing about coordination or reading difficulties, and can spend years working above foundations nobody ever checked. Ask yourself which of those categories your own training left out.
When someone cannot let go of the old rule after the rules have changed, that is the same machinery that loses their keys and drops the thread of a sentence. Stuck, not stubborn.
