A structured set of questionnaires covering childhood, school, work and daily life turns vague instability into something countable, and it prompts questions you would otherwise forget to ask. Gather more than one perspective where you can, since a parent, a partner and the person themselves will each notice different things. Keep the completed forms, because they also give you a baseline to compare against later.
You can ask those who have knowledge if you do not know (Quran 16:43). Working from what has been carefully built by others is the pattern being commended there.

A long term follow up of 515 children into young adulthood found that reports of how a young person was doing differed depending on who was asked. That is a direct argument for collecting more than one account rather than trusting a single impression. It was a follow up study rather than a comparison of assessment methods.

Strength of evidence: one line of research. A single study, or a theoretical account.
A standard set of questions asks the same things every time, which is what makes patterns visible rather than memorable.
If sport or a club is the reason a struggling student still turns up, do not let it be the first thing taken away when grades slip. Removing it usually removes what was holding the rest together.
Far more people have ADHD than are ever assessed for it. If you have wondered about yourself for years, the odds that an assessment would be worth your time are better than you think.
Being able to disappear into something for six hours does not cancel the diagnosis out. Getting stuck on and getting stuck off are the same switch failing in two directions.
