This turns up most often where there was one parent and nobody else to keep watch. The person cannot settle, cannot stay with a task, and has assumed for years that their attention is simply broken. Getting the history out changes what you offer them, and it often changes how they see themselves, because watchfulness learned for a reason is far easier to live with than a defect.
The soul was 'inspired to know its own rebellion and piety' (Quran 91:8). The capacity to recognise what is actually going on inside you is given, and this is one small use of it.

A review of adults presenting to services with other psychiatric problems found unrecognised ADHD underneath them fairly often, partly because symptoms overlap between conditions and partly because people do not reliably see their own attention difficulties. It is a narrative review, so treat it as clinical argument. The traffic runs both ways: what looks like ADHD may have come from somewhere else entirely, and the history is usually what tells you.

Strength of evidence: one line of research. A single study, or a theoretical account.
Attention that was trained to stay on the door is not attention that has failed.
If what surfaces is frightening rather than merely sad, slow the pace and make sure there is proper support around the person before going further.
It runs the other way as well. Among children referred for dyslexia, a sizeable minority also have ADHD and many have coordination difficulties too. Whatever is written on the referral, treat it as one part of the picture.
When you read that some proportion of people with ADHD have a particular problem, the number tells you nothing until you know the figure for everybody else.
When a child with attention difficulties is also at war with every instruction, that pairing is worth taking to someone early rather than waiting to see whether it settles.
