What a setting asks for is the ratio between the self control a person has to supply and the structure being supplied for them. One adult, close attention, interesting things and quick responses covers most of the gap. A room where a child must wait, sit, follow three instructions and hold on for an hour does not cover any of it. So he is fine with me is not evidence against the diagnosis, and it can be the most damaging sentence a family hears.

So truly where there is hardship there is also ease (Quran 94:5). Hardship and ease sitting side by side is closer to the truth here than a person being one way all the time.

A systematic review of bupropion in children and adolescents was undertaken partly because up to twenty percent of patients do not respond to stimulants and stimulants can worsen sleep, mood or anxiety in some. Variation of that size is taken as normal in this field rather than as a reason to doubt the diagnosis. That review is about differences between people in drug response, not about the same child across different rooms, so the parallel is a loose one.

The difficulty shows up in proportion to how much a setting asks a person to regulate themselves without help.
After a working lifetime in this field, Barkley names the change in how a family sees the child as the most important step of all. Not the medication, not the charts.
A lot of what people now know about ADHD came from a video. Some of it is genuinely good, some of it is not, and asking someone what they have been watching is a fair question to open with.
The point of treatment is not a tidier score on a questionnaire. It is fewer accidents, a finished course, work that holds, relationships that survive, a longer and better life.
