They ask for different things. The agitated version often tolerates and even welcomes something sedating, while the flattened version needs whatever gets a person moving and is made worse by anything that adds weight to the eyelids. Most people carry a mixture, so the question is which one is doing the damage now. Asking about the last fortnight in particular gets a better answer than asking about the illness in general.
'When We give people a taste of Our blessing, they rejoice' (Quran 30:36), and the same verse describes them falling into utter despair when trouble arrives. Two very different states in one person, named separately.

A research review of adolescent depression sets out the evidence that a blunted response to reward is a core feature of depression rather than only a by-product of feeling low. It reviews a body of work largely in young people and is not a treatment finding. It does support treating the pleasureless, flattened presentation as its own thing rather than as a gentler version of the same picture.

Strength of evidence: one line of research. A single study, or a theoretical account.
What helps depends on which direction the person needs shifting in.
The newer thinking about depression is less about topping up a missing chemical and more about connections that have thinned out being rebuilt. Recovery looks more like regrowth than refilling.
Ask why the tablets take weeks. That one question sorts the honest accounts of how they work from the tidy story about a chemical being low.
Stopping suddenly can bring lethargy, nausea, irritability and headache within a few days. That is discontinuation, not the illness coming back.
