Move the sign about. Leave the gap in the middle, four plus something equals two plus five, and watch what the child does with it. If they add everything up and write eleven, you have found something well worth fixing now, because that misreading sits underneath everything algebra will later ask of them. It takes minutes at seven and years at fourteen.
We have knowledge only of what You have taught us (Quran 2:32). If nobody has told a child what the sign means, they have invented a rule to fill the gap, and inventing one was reasonable of them.

There is no study of the equals sign among the works gathered here, so the reasoning is educational rather than experimental. What the research does support, in a systematic review of self-regulated learning, is that how learners plan, monitor and check their own understanding is consistently linked to how they do, and that these habits are changeable rather than fixed. Stopping to ask what a symbol actually means is that kind of checking, made concrete.

Strength of evidence: one line of research. A single study, or a theoretical account.
If the sign is read as an instruction rather than a statement of sameness, every equation with a gap in the middle goes wrong.
When reading results lift across a whole state after every teacher is retrained, the lesson is uncomfortable and hopeful at once. A good part of what looks like children failing is teaching that has not caught up yet.
In practice, you sometimes have to stop a child using the trick that has kept them afloat, so the weaker skill finally gets used. Expect them to hate it for a while.
Phonological awareness is not one skill. Telling sounds apart, rhyming, breaking a word into its sounds and blending them back are separate jobs, and a child can be fine at one and stuck at another.
