They act on receptors at the front of the brain that support working memory and holding a response back, which is a different route from the reuptake blocking that stimulants use. Nobody claims the mechanism is fully mapped. Saying that plainly to someone who wants to understand what they are taking works better than a tidy story, because partial knowledge honestly described is easier to trust.

'Lord, increase me in knowledge!' (Quran 20:114), said alongside an instruction not to rush ahead of what has been given. Understanding a treatment can go on honestly while parts of it remain unknown.

Where head-to-head trials do not exist, reviewers have compared adult ADHD medicines indirectly to give some basis for ranking the options. Indirect comparison is weaker than putting two drugs against each other in one trial, and the authors offer it as a working basis rather than a settled answer. So the ordering of these medicines rests on assembled evidence rather than on direct contest.

Strength of evidence: one line of research. A single study, or a theoretical account.
A different route into the same circuitry can help where the usual route cannot be used.
When you are weighing up whether a teenager is ready for something, it helps to think in two thirds. A fifteen year old may be handling responsibility more like a twelve or thirteen year old.
The line about not cutting, crushing or chewing a long acting medicine is a safety instruction with a real mechanism behind it. Break the coating and the whole day's dose arrives in one go.
These medicines do not pour a foreign chemical into you. Mostly they help you hold on to the serotonin your body already makes, so whatever follows is yours.
