Areas › Focus, starting, and ADHD › Medication and how it works
Knowing this changes what you expect from it. The rise is gentle rather than sudden, the afternoon is deliberately given a slightly stronger push because the body responds less as the hours pass, and the empty shell sometimes appears intact afterwards, which is normal and does not mean the dose was lost. Crushing or chewing one of these breaks the whole design, so they go down whole.
This explains how treatments work. Never start, stop or change a medication because of something you read here.
'He who cures me when I am ill' (Quran 26:80). Abraham puts the cure inside his description of God, which leaves plenty of room for taking a carefully made remedy seriously.

What a delivery system is buying you is medicine present at the moments that need it. A systematic review and meta analysis of imaging studies found that stimulants consistently settled activity in particular frontal and striatal regions during tasks that call for attention and for holding back a response. Those studies looked at brain activity rather than at capsules and school days, so read them as an account of how the drug acts, not as evidence that any one product will suit you.

Strength of evidence: one line of research. A single study, or a theoretical account.
The medicine is let out at a steady rate through the day instead of arriving all at once and then draining away.
If swallowing capsules whole is not possible, ask about a different formulation rather than opening this one.
A plain immediate release tablet works for roughly three to five hours, so an ordinary school day needs two or three of them. If the afternoons keep falling apart, that gap is worth naming out loud with the prescriber rather than reading it as failure.
Dry mouth, a later bedtime and a smaller appetite are the usual early complaints, and each has a workaround. Knowing about them in advance turns a nasty surprise into something you were already expecting.
Smoking runs high in ADHD, and nicotine genuinely does something for attention. It helps to treat that as self-medication rather than as one more impulsive habit.
