With a narrow window, things that would be trivial elsewhere start to matter: a bout of dehydration in hot weather, a course of anti inflammatory painkillers, a blood pressure tablet added by another doctor, a brand switched at the pharmacy. Any of them can shift the level. It is worth asking anyone on such a medicine to mention it every time another prescription is written, including at the dentist.

Measuring rather than guessing is not reserved for the dramatic drugs. A meta analysis of stimulant treatment in adults found small but consistent rises in heart rate and blood pressure. The effect was modest for most people, which is precisely why it needs a cuff rather than an impression, and stimulants are a very different class from lithium. The shared lesson is that a physical measure tells you something a conversation cannot.

Strength of evidence: one line of research. A single study, or a theoretical account.
When the helpful amount and the harmful amount sit close together, anything that nudges the level can push a person across the line.
Coarse tremor, vomiting or confusion in someone taking lithium needs urgent medical attention rather than a wait and see.
Some settings simply suit an attention difficulty better. Expectations set out minute by minute, consequences that arrive the same day, a visible ladder to climb: military life often fits for exactly those reasons.
The question that usually matters most is not which molecule but how many hours it covers. A medicine that has worn off by four in the afternoon leaves the hardest stretch of the day uncovered.
Most people who try do find something that helps, but often not the first thing they try. Not responding to one class of stimulant says surprisingly little about the other.
