Name it before you begin and write it at the top of the page. Keep it close: this evening rather than next term. It does not have to be impressive. The match, a bath, the programme you are saving up. If the only thing waiting at the end is a grade in June or a vague sense of having been good, there is nothing pulling at you during the difficult middle hour, which is precisely when the pull is needed.
Each gains whatever good it has done (Quran 2:286). Effort is described as counting towards something, and keeping that something in sight is what makes it bear on the afternoon you are in.

لَا يُكَلِّفُ ٱللَّهُ نَفۡسًا إِلَّا وُسۡعَهَاۚ لَهَا مَا كَسَبَتۡ وَعَلَيۡهَا مَا ٱكۡتَسَبَتۡۗ رَبَّنَا لَا تُؤَاخِذۡنَآ إِن نَّسِينَآ أَوۡ أَخۡطَأۡنَاۚ رَبَّنَا وَلَا تَحۡمِلۡ عَلَيۡنَآ إِصۡرࣰ ا كَمَا حَمَلۡتَهُۥ عَلَى ٱلَّذِينَ مِن قَبۡلِنَاۚ رَبَّنَا وَلَا تُحَمِّلۡنَا مَا لَا طَاقَةَ لَنَا بِهِۦۖ وَٱعۡفُ عَنَّا وَٱغۡفِرۡ لَنَا وَٱرۡحَمۡنَآۚ أَنتَ مَوۡلَىٰنَا فَٱنصُرۡنَا عَلَى ٱلۡقَوۡمِ ٱلۡكَٰفِرِينَ
God does not burden any soul with more than it can bear: each gains whatever good it has done, and suffers its bad- ‘ Lord, do not take us to task if we forget or make mistakes. Lord, do not burden us as You burdened those before us. Lord, do not burden us with more than we have strength to bear. Pardon us, forgive us, and have mercy on us. You are our Protector, so help us against the disbelievers.’

A systematic review confirmed that putting things off co-occurs with ADHD often enough to be a genuine pattern, while noting that the mechanisms behind it remain unclear. The account offered here, that distant rewards fade out of view, is one of several proposed explanations rather than a settled one. That makes this a reasonable thing to try on yourself, with your own weeks as the evidence.

Strength of evidence: one line of research. A single study, or a theoretical account.
A reward too far ahead stops registering, so bringing it nearer is what gives the effort something to lean towards.
With attention difficulties the organising side often runs a few years behind the birthday. A nineteen year old may be running life more like a sixteen year old, and that is a lag rather than a verdict.
Difficult things come out more easily when your hands are occupied and you are not looking straight at the other person. Walk side by side, drive somewhere, wash up together, keep the sewing going.
Every small snag in a task is an exit. The forgotten password, the missing form, the bit where you are not sure what happens next: each one is a chance to say forget it, I will do this later.
