So ask directly, and ask about sleep while you are there. People do not usually announce these things. They quietly stop taking the tablet and say later that it did not suit them. Most of these effects come from serotonin receptors that have nothing to do with mood, which is why the gut gets involved at all, and several of them settle over the first few weeks.
The Qur'an pictures a breast closed and constricted 'as if they were climbing up to the skies' (Quran 6:125). Bodily language for an inner state is scriptural, so describing what your body is doing is a fair thing to bring to an appointment.
الأنعام

A review of sleep and circadian function in mood disorders argues that disturbed sleep is not merely a symptom of low mood but contributes to episodes, to relapse and to poorer emotion regulation. It is a narrative review rather than a trial, so take it as a well argued case rather than a settled finding. It does make the insomnia line on a side-effect list worth more attention than it usually gets.

Strength of evidence: one line of research. A single study, or a theoretical account.
Something named out loud can be managed, while something nobody asks about tends to end the treatment.
If pain is part of what you are carrying, bring it into the same conversation as the low mood. They are not two separate appointments.
If the usual antidepressants have not reached you, there are treatments that work on an entirely different system in the brain. They are worth asking about.
Some newer antidepressants do two jobs in a single tablet: the familiar one of keeping serotonin available, plus a direct nudge at the receiving end.
