The reason is unglamorous. Nearly every strategy anyone teaches assumes you can pause before responding, hold a plan in mind while doing something else, and put up with a bit of delay. Those are exactly the capacities that are thin here. Medication tends to lift them a little, and everything built on top of them works better once it has.
This explains how treatments work. Never start, stop or change a medication because of something you read here.
Abraham described God as the one 'who cures me when I am ill' (Quran 26:80). Seeking a cure is placed inside faith there, not set against it.

A systematic review with network meta analysis of randomised trials in children and adolescents found stimulant medication produced the largest symptom reductions of any single intervention, with non pharmacological options doing less well on the same outcome measures. Symptom scales are not the whole of a life, and that review covers young people rather than adults. Within what it actually measured, the finding is consistent rather than marginal.

Skills that depend on being able to pause and hold a plan work better once that capacity is a little stronger.
Medication is a decision for the person and their prescriber, and pushing someone who has already said no usually closes the conversation for good.
Some capsules are tiny pumps. A coating of powder gives an early lift, then water seeps in and slowly pushes a gel of medicine out through a laser drilled hole, with a little more strength held back for later in the day.
Long acting stimulants are the usual starting point for a plain reason: they cover the most hours and they remove the lunchtime dose, which is the dose ADHD is most likely to swallow whole.
Two products can hold the identical molecule and still behave differently in a person, because the difference lives in how fast the drug is released. A change of formulation is a real change, and it is a different thing from a change of drug.
