At a dose that suits them, most people can say when it comes on, what the middle of the day feels like, and when it leaves. Vagueness points somewhere specific: too low a dose, the wrong form, or something else going on that this medicine was never going to touch. All three can be acted on, so treat the shrug as information rather than as a mild success.
This explains how treatments work. Never start, stop or change a medication because of something you read here.

Doses in adults are often left below what the trials used. A review of treatments for adult ADHD reports that effect sizes for stimulants approach those seen in school age children when adequate doses are used, with atomoxetine and bupropion as second line options. It is a narrative review rather than a pooled analysis, and what counts as adequate is settled with a prescriber and not from a chart. The point still stands that under dosing is a common and fixable reason for a disappointing result.

Strength of evidence: one line of research. A single study, or a theoretical account.
A change big enough to help is usually big enough to notice.
Dose changes belong with the prescriber, so this question is for gathering information rather than acting on it.
Pill swallowing can be taught in a few short sessions with sweets. Start with the tiniest ones, give two to eat for every one swallowed whole, and work up in size across a week.
Medication treats the attention difficulty. It does not teach reading, lift a depression, or repair a household that has been fighting for years.
With many of these medicines the unpleasant part turns up on day one and the good part takes weeks. Knowing that in advance is most of what carries people across the gap.
